Consent to Drug and Alcohol Testing  

I, ____________, understand that Haddad's Market prides itself in maintaining a safe work environment for its employees and others and accordingly requires all employees to consent to a random urine test for drug and/or alcohol use.

I hereby freely and voluntarily give my consent to such test, to be administered at any time during my employment, having been fully informed of the test procedure involved, of what I am being tested for, and that a copy of the results will be forwarded to Haddads' Market and placed in my record. The employer will hold this information in strict confidence. I also understand that, in the event of a positive test result, Haddads' Market will give me an opportunity to explain such result.
